Terms of Service

These terms outline the rights, responsibilities, and expectations for individuals receiving residential support services at Aeternum Care

1. Service Agreement

By accepting placement and receiving residential support services at Aeternum Care, you (or your legal guardian/representative) agree to these Terms of Service. These terms govern the provision of services for individuals with intellectual/developmental disabilities (I/DD) and mental health needs in accordance with Oregon state regulations.

These terms are designed to ensure a safe, supportive, and respectful environment for all residents and staff.

2. Resident Rights

All individuals receiving services at Aeternum Care have fundamental rights protected by Oregon law and federal regulations:

  • Right to dignity, respect, and person-centered care
  • Right to privacy and confidentiality of personal information
  • Right to participate in service planning and decision-making
  • Right to refuse services or treatment
  • Right to file grievances without fear of retaliation
  • Right to freedom from abuse, neglect, and exploitation
  • Right to communicate freely and have visitors
  • Right to manage personal finances (when capable)

3. Resident Responsibilities

To maintain a safe and supportive community, residents are expected to:

  • Treat staff, other residents, and visitors with respect
  • Follow house rules and community guidelines
  • Participate in person-centered planning and goal-setting
  • Communicate needs, concerns, and preferences to staff
  • Take prescribed medications as directed (when applicable)
  • Attend scheduled appointments and activities
  • Respect the property and personal belongings of others
Important Notice

Failure to follow community guidelines may result in additional support planning or, in serious cases, discharge planning in accordance with Oregon regulations.

4. Prohibited Behaviors

To ensure the safety and well-being of all residents and staff, the following behaviors are prohibited:

Violence, threats, or aggressive behavior toward others
Possession or use of illegal substances or weapons
Theft or destruction of property
Sexual harassment or inappropriate conduct
Discrimination or harassment based on protected characteristics

5. Service Scope and Limitations

Aeternum Care provides residential support services within the scope of our licensing and staff qualifications:

Our services include:

  • 24-hour supervision and support in a residential setting
  • Assistance with activities of daily living
  • Medication administration and monitoring
  • Coordination with healthcare providers and case managers
  • Life skills development and community integration

We do not provide acute medical care, psychiatric hospitalization, or services requiring specialized medical licensing beyond our scope.

6. Grievance and Appeals Process

Residents and their representatives have the right to file grievances regarding services, treatment, or any concerns. We maintain a formal grievance process that includes:

  • Submission of written or verbal complaints to program management
  • Investigation and response within required timeframes
  • Right to appeal decisions to licensing authorities
  • Protection from retaliation for filing complaints

Contact information for filing grievances is posted in common areas and provided during admission.

7. Financial Responsibilities

Payment for services is typically covered through Medicaid, private insurance, or other funding sources. Residents or their representatives are responsible for:

  • Providing accurate insurance and eligibility information
  • Notifying us of changes in coverage or funding
  • Payment of any applicable co-pays or non-covered expenses
  • Personal expenses not covered by the service agreement

We will work with you to identify funding sources and maintain continuity of care.

8. Discharge Planning

Discharge from services may occur for various reasons and will be conducted in accordance with Oregon regulations:

  • Voluntary discharge at the request of the resident or guardian
  • Achievement of service goals and transition to less restrictive setting
  • Change in needs requiring a different level of care
  • Serious violations of community rules affecting safety

All discharges will include appropriate planning, notice, and coordination with case managers and receiving providers to ensure continuity of care.

9. Governing Law and Regulations

These Terms and the provision of services are governed by:

  • Oregon Administrative Rules (OAR) for residential support services
  • Oregon Health Authority licensing requirements
  • Federal HIPAA privacy and security regulations
  • Americans with Disabilities Act (ADA)
  • Other applicable federal and state laws

10. Changes to Terms

We reserve the right to modify or replace these Terms at any time. We will provide notice of any changes by posting the new Terms on this page.

By continuing to access or use our service after those revisions become effective, you agree to be bound by the revised terms.
